What might cause my car to not respond to acceleration occasionally after stop or coasting?

'94 Jeep GC, seldom driven, 145K miles, recent tune up and safety inspection. Seems to only happen in stop/go traffic - the engine will rev as if in Neutral, then seems to 'catch' that it's in Drive. Car will lurch but then drive smoothly.
